Requirements
- Doctor of Pharmacy
- Minimum 3 years of experience in an acute care hospital, preferably in JCI/NABH accredited hospitals
- Registered pharmacist at a licensing authority or regulatory body
- Accuracy and critical thinking
- Effective communication and interpersonal skills
- Proven ability to work effectively in a team environment and independently as required
Responsibilities
- 01Promote rational medicine therapy that is safe and appropriate
- 02Evaluate and monitor prescriptions to optimise medicine therapy according to Evidence Based Medicine
- 03Evaluate drug-drug interactions and support dose adjustments based on renal and hepatic function, patient-specific factors and approved treatment protocols
- 04Provide a comprehensive clinical pharmacy service hospital wide
- 05Identify medication errors, including prescription, transcription, administration and dispensing errors, with daily reporting
- 06Support drug information services
- 07Contribute to medication safety initiatives and quality improvement projects
- 08Support antimicrobial stewardship and rational antibiotic use
- 09Assist in pharmacovigilance activities, including adverse event reporting and medication safety monitoring
